STH Dirty TricksSheffield Teaching Hospital Trust Contravened 'Access to Medical Records'.1. 90 days after we applied and paid for copies of the Medical Records, STH Palliative Care Consultant said we couldn't have them (delaying tactic to take us out of timescale for approaching the Healthcare Commission). However we went up to the Medical Records office and a clerk, went straight to a bottom drawer of a desk and produced a dishevelled pile of medical records that were all out of sync! (Probably heading for the shredder). 2. 4 years after the death of Thomas and we find that the STH had not given us all the Medical Records in the first place. For some reason they send us 16/12/09 the Dangerous Drug Register and other drug charts. (See 'Our Evidence' catagory where these drug charts can be seen). 3. Our Solicitor confirms January 2010 that although she had written to the Sheffield Teaching Hospital Trust 2007 -2009, to ask if they had any other medical records or drug chart, the STH and said that our Solicitor had all the medical records. STH were not telling the truth! Contravened their own guidelines and protocol in the administering of Morphine and Contravened 'Misuse of Dangerous Drug Regulations' and 'Misuse of Dangerous Drug Act 1971'. Dangerous Drug Chart entries had been alterered. We have seen the original Drug Charts and Medical Records 2010 and it was clear that alterations had been made. Alteration should not be made and STH + National guidelines say that if a 'mistake' has been made, a line should go through the entry and a fresh entry made. (The STH were trying to pretend that a syringe driver had been refilled at 20.20p.m. 10/1/06, although in the Syringe Driver infusion chart they say that the refill was at 22.15p.m., - 22.15p.m. is nowhere in the Dangerous Drug Register Both charts can be seen under 'Our Evidence' catagory. |
