Or to remove any of the documents that my Independant Witness and myself viewed yesterday. My Independant Witness and myself look upon the method of trying to stop us from seeing the original DDA/CD Register to have been a continuation of the shambolic way in which my father was treated in the first instance and the now continued poor treatment of his family. By saying the photostat copy that you produced was a true copy of the original was not acceptable. Once we were granted permission, late afternoon, to view the redacted DDA/CD chart, it was clear and apparent to all of us there present that the entry 20.20p.m. 10/1/06 had been altered. This contravenes your Hospital guidelines and we look upon this as a serious matter. I must make you aware that at NO other time has a syringe driver nor subcutaneous injection been noted neither in listed times of medication given sent to you from my solicitor, which you could have questioned, or anywhere else in any other list. This time appeared for the first time in the erroneous letter of Sir Andrew Cash OBE to Mr. David Willett's MP 14/8/09. Dr. Mike Richmond who telephoned me 24/9/09, once I had pointed out the errors, could NOT stand by the times as noted in my email to him, BUT now an altered record supports a time in Sir Andrew's list. Email regarding Dr. Mike Richmonds remarks is below. Your inability to comment on the obvous alteration has been noted. I took a photograph of this alteration which is now in my possession. I also put the Sheffield Teaching Hospital Trust on Notice that: 1. You deliberately withheld drug charts from my solicitor 2007 to present date, although she has confirmed to me that she requested FULL disclosure of all medical records. You have now newly disclosed to me 16/12/09, Graesby Observation Charts and DDA/CD Chart here attached. And some multidisciplinary Staff signatures. 1a. We formally note however why in these Graesby Charts you could NOT explain the omission a supposed set up of a syringe driver, 20.20p.m. 10/1/06. 1b. We formally note that you could NOT explain the grave discrepancy whereby in the DDA/CD chart 8.45a.m 11/1/06 2.5mgs. morphine is recorded but in the PRN chart previously disclosed to us 5mgs. morphine is recorded. 2. I put you on notice that the NHS Litigation Service wrote in 2008 that NO type of Observation Charts had been made on the MPCU. 3.You have deliberately destroyed the DDA/CD chart for the 7th and 8th January. Your letter to me 24/1/06 stated that my enquiry had been passed on as a COMPLAINT. This should have meant that any medical records pertaining to my father should have been kept. 4.You have NOT disclosed the Multidisciplinary Staff Signature chart for the period 9th, 10th and 11th January, 2006. I formally yet once again request a copy of this. We requested this again yesterday at the same time as the original DDA/CD Chart, but once again this was not forthcoming. 5.You have not disclosed the names of nurses included on the Duty Rota for the period 9th, 10th and 11th January, 2006. Once again yesterday you failed to supply the original of this document. We note that ALL the nurses names including those of the student nurses on duty a.m. 11/1/06 had been obliterated. In order for us to clarify dates and times when medication was given it is of great importance that this list be passed to us. Seeing the nurses names does not contravene patient confidentiality. I formally put the Sheffield Teaching Hospital Trust on Notice that you are continuing to try to mislead me as you mislead my M.P. Mr. David Willetts, when you wrote to him 14/8/09 saying that the set up/commencement time of the morphine syringe driver was at 22.50p.m. 9/1/06 but now have conceded that it was in fact 1.50a.m. 9/1/06. This was an attempt as in noted in the letter 14/8/09 to try to say there wasn't enought time to calculate and consider any prn that had been required in preceding 24 hour period. But as we all know there was plenty of time to calculate and consider. As you should be also aware the parental dose of morphine was established by 8th January when my father NEEDED 30mgs. prn morphine and this should have indicated the starting dose in the morphine syringe driver at 30mgs and NOT 10mgs which happened. I would appreciate full disclosure of the above still 'missing documents' but if not I will be calling upon various acts and guidelines if needed.
