Request the Medical Records under the Access to Medical Records Act 1990
Click here to view the Department of Health Information on how to access Medical Records

As SOON as you have the Medical Record copies request to view the originals either at the NHS Hospital or NHS GP Surgery. You will then be able to compare and make sure that you have ALL the necessary documents and you will be able to note any alterations. You may have to request this under the Data Protection Act or the Freedom of Information Act if the NHS establishment are trying to avoid letting you see the originals.

If you are concerned about the care given to a loved one make sure that you obtain the Duty Rota to make sure the staff that were involved had the correct qualifications!

Always ask for the Hospitals guidelines and protocol on the particular treatment being given. For us is was the Palliative Care Guidelines in force at Sheffield's Northern General Hospital.

If you find any anomalies in the drug charts or dangerous drug records the 'Misuse of Drugs Regulations 2001" should be consulted. Click here to read the 'Misuse of Drugs Regulations 2001" Document.
