Falsified Documents?

26/1/10 - We have now seen some original NHS Medical documents.  The Dangerous Drugs Book had been physically altered.   This is serious.

Also a mistmatched amount of morphine - 2.5mgs. morphine entered in to the Dangerous Drugs Book (DDA) 8.45a.m 11/1/06 does not match the 5mgs. morphine entered in to the prn as required morphine chart.  NHS staff and the NHS Deputy Chief Nursing Officer, Mr. Richard Parker, could not give us an explanation for this!

JUST WHAT HAVE THEY BEEN UP TO?

22/1/10 - Yet another email sent to Andrew Cash and his colleagues requesting that newly disclosed documents be signed as being accurate and having been completed during the 7/1/06-11/1/06 Time Frame.  Result = Silence.

I can confirm that I now have the hard copies of the documents that were originally sent to me via email 16/12/09.
 
Unfortunately I do not have the 'missing documents' that I have repeatedly requested be supplied to me by the Sheffield Teaching Hospital Trust.
 
1. The DDA (MAU CDA) listings for the 7th and 8th January, 2006.
2. A FULL set of the 'Multidisciplinary Staff Signatures'.
3. Sheffield Teaching Hospital Guidelines or Protocol on the duration of DNAR's
 
Outstanding questions requiring answers.
 
a. I require clarification of whether the DDA entry timed at 22.50 9/1/06 has an S/D or S/C and also, as the first entry has a wasted amount listed underneath it seems to follow that the entry timed 22.50 is likely to have the wasted amount beneath it too.

b. There is an entry timed at 20.20p.m. of 10mgs shown in the DDA chart on 10/1/06.  Can you confirm for me exactly what this corresponds to.  Is it a syringe driver refill, a subcutaneous morphine injection or an injection of midazolam?

Could you be so kind as to list clearly these dates, times and amounts given (DDA) and sign them to confirm that they are accurate please?  

I would be grateful if you could send me, via email, a signed acknowledgement that the Graesby Observation Charts that you have now disclosed 16/12/09 are accurate and were completed during the 7/1/06 -11/1/06 time frame.
 
The Graesby Observation Charts disclosed to me in the first instance 16/12/09, were NOT disclosed to my Solicitor at any time. I would be grateful for an explanation as to why?
 
I respectfully request that a declaration is completed by someone who is 'in a position to verify clinical documents'.
If the above declaration cannot be provided then I will have no alternative than to assume they are not accurate and completed during the 7/1/06 - 11/1/06 time frame.

N.B. The documents (Graesby Chart and DDA - Dangerous Drug Charts) that were passed to us 16/12/09 have NEVER been disclosed before neither to us nor to our Solicitor.  These charts confirm dates listed by Sir Andrew Cash OBE of when medication was given. Strangely enough when Dr. Mike Richmond phoned us 24/9/09 he said the Sheffield Teaching Hospital Trust would NOT stand by Sir Andrew's list!

24/9/09 nearly 4 years after the death of Thomas Milner and Dr. Mike Richmond, Medical Director of Sheffield Teaching Hospital Trust which includes the Sheffield Northern General Hospital, makes  contact via telephone - Why Now?    

We had received an erroneous letter a few days earlier from Sir Andrew Cash OBE, Chief Executive of Sheffield Teaching Hospital Trust!  In this letter Andrew Cash lists times of set up and refill of the pain control morphineSyringe Driver. 

Where did Sir Andrew Cash OBE, Chief Executive of Sheffield Teaching Hospital Trust get his information. WE HAVE THE MEDICAL NOTES, we know when and what pain control was and was not given!

N.B. Dr. Mike Richmond, WOULD NOT STAND BY times listed in Sir Andrew's letter!

Thankyou for taking the time to telephone me this afternoon. (24/9/09)
 
I was dissappointed that you refused to give me your email address but your Secretary was very helpful. You too now will be copied into any further communications.
 
You have offered to meet with me regarding 'I don't know what'.   You seemed unable to address my concerns as to what there was to discuss at this late time in this protracted case!   A decent apology for ALL the errors made by the Sheffield Hospital Trust including Sir Andrew Cash's incorrect letter really seems the only decent thing to be done.      I did however leave your offer open.
 
I can confirm that I reminded you about the ACTUAL time of the 'set up' of my father's morphine syringe driver - 2.00a.m. 9/1/06 on the MAU ward of the NHS Northern General Hospital, Sheffield.
 
I can confirm also that I reminded you that Sir Andrew Cash OBE's letter said quite clearly that 'any as required medication that has been administered in a preceeding 24 hour period should be calculated and considered into the next syringe driver refill'. (This was not done by the Palliative Care Team at the Northern General Hospital, Sheffield.)
 
I can confirm that I reminded you that whilst on the NHS Palliative Care ward alone, from admittance (notes confirm syringe driver in situ) 18.00p.m. 9/1/06, until 8.10a.m. 10/1/06 (14 hours) my father had NEEDED 6 x 5mgs. (30mgs) morphine and midazolam prn.  I also mentioned to you that from the set up of the syringe driver, 2.00a.m. 9/1/06 on the MAU ward until admittance to the Palliative Care ward, Northern General Hospital, Sheffield, 18.00p.m.9/1/06, my father had also NEEDED several subcutaneous morphine injections (prn).
 
I can confirm that I reminded you that by the time the Palliative Care Consultant did her round 10.30a.m. 10/1/06 the syringe driver had been in situ for 32 hours.   This Palliative Care Dr. failed to 'calculate and consider' any prn that had been NEEDED in the preceeding 24 hour period. (She left the syringe driver holding just 10mgs. morphine.)
 
I asked you to confirm that the Northern General Hospital and Sheffield Teaching Hospital Trust was standing by the times of the set up of the syringe driver and the refills as listed in Sir Andrew' Cash's letter  - BUT you  Dr. Mike Richmond could NOT do this.
 
When you mentioned the letter as being a possible 'forgery', I was surprised.
 
I can confirm that I telephoned the secretary of Sir Andrew Cash OBE 21/9/09 to ask who had written the unsigned letter that had been sent to my Hampshire M.P. in the name of SIR ANDREW CASH OBE, Chief Executive of the Sheffield Teaching Hospital's Trust as I was concerned that the times of set up and refills of the morphine syringe driver were ERRONEOUS.     I was passed onto the Complaint's Department at the Hospital in order to speak to a Complaints Manager.   I was told someone would call me back 21/9/09 which they failed to do.
 
I attach the letter from Andrew Cash along with my letter which has been sent to Gordon Brown Prime Minister, Christine Beasley, Chief Nursing Officer, Liam Donaldson, Chief Medical Officer, Andy Burnham M.P., and Minister for Health, David Willetts M.P., Lady Finlay of Llandaff, Lord Joffe, Lord Turnbull as well as other 'interested parties.
 
May I remind you that your deputy Dr. Throssell emailed me 27/8/09 asking for time to consider the questions put to Mr. Stone, Chairman of the Board, Sheffield Teaching Hospital Trust,  by my UKIP-MEP (I attach this letter for your convenience).  Up to today's date I have had no further communication from Dr. Throssell or Mr. Stone. 
 
I will be at a Healthcare meeting in Brighton next week by invitation and am meeting with Journalists also regarding this matter.
 
I await any further comments you may wish to make.
