Another Letter to Sir Andrew Cash OBE 26/10/09


Dear Sir Andrew,

I remain perplexed in view of your letter dated 30/9/09 notwithstanding your letter dated 14/8/09 addressed to my M.P. David Willetts.
When you mention my ‘additional’ correspondence of the 22/9/09, are you referring to the email that I copy and paste here below – for ease of reference or to our letter 22/9/09? (The letter can be viewed at 'Our Reply to Andrew Cash' under CASE POINTS)

‘I have now spoken to the office of Sir Andrew Cash OBE and nobody seems to know who wrote the letter attached (hospital letter)! (See CASE POINTS 'Letter from Sir Andrew Cash OBE')

This letter was sent to David Willetts M.P. 14/8/09 in the name of Sir Andrew!!! Interestingly the Hospital sent out a very different letter to my other M.P. Mr.Richard Cabourn 5/8/09 - that letter was just photocopies of Hospital correspondence to me over the last nearly FOUR very long years. Which needlessly to say these letters were full of discrepancies v. the medical notes! Some of which are outlined in the attached (Final Letter). (See CASE POINTS 'NHS Trauma 7/1/06 - 16/10/09')

Who is running this 'plot'. Is it the 'men in suits' or just some other bod?

May I remind you that a lot of misleading comments have been made to the Press.

I wonder what will be made of this 'GAFFE'?

As you will see below in my comments to David Willetts this 'latest' letter is worrying and leads one to question the competance from the lowest level to the highest!

You also say that from the letters I have sent you ‘it appears’ that I have ‘decided to refer (my) concerns to the Nursing and Midwifery Council, the General Medical Council and the Parliamentary and Health Service Ombudsman’.
Could you clarify the above statement? NOWHERE in any of my correspondence to the Prime Minister, M.P.’s Ladies and Lords etc., that I have also sent you copies of, does it mention that I would be going to ANY of the above advisory bodies.

I once again ask you – AS THIS MATTER IS NOT WITH ANY OTHER NHS DEPARTMENT – to explain your ERRONEOUS writings that I copy and paste here for ease of reference:

INCORRECT Medication Details from Sir Andrew Cash:

9/1/06 22.50 - Syringe Driver set up (10mgs. 24 hours) - Palliative Care ward
10/1/06 20.20 - Syringe Driver refilled (10mgs. 24 hours)

11/1/06 09.40 - Syringe Driver refilled (doubled) (20mgs. 24 hours)

The set up time is wrong as is the re-fill time -

FACTUAL Medication Details from the Medical Notes:

9/1/06 2.00a.m. - Syringe Driver set up (10 mgs. 24 hours) - MAU Ward

10/1/06 06.40a.m. - Syringe Driver refilled (10mgs. 24 hours)

11/1/06 09.40a.m. - Syringe Driver DOUBLED (20mgs. 24 hours)

Can you explain the ERRONEOUS, INCORRECT, MISLEADING LISTINGS?
Can you explain also why I, nor my UKIP-MEP, Nigel Farage, have NOT had a reply to the 3 questions he posed on my behalf, 19/8/09, to Mr. Stone, Chairman, Sheffield NHS Teaching Hospital Trust?

N.B. Dr. David Throssell, Deputy Medical Director of the Sheffield NHS Teaching Hospital Trust emailed me 27/8/09 asking for more time to consider these questions. I nor Mr. Farage have had a reply. Dr. Throssell’s email is here for ease of reference:

Thank you for your e-mail, in which you request a response to three questions raised in a letter recently sent to Mr David Stone, Chairman of the Sheffield Teaching Hospitals NHS Foundation Trust. Clearly it is important that the response to this letter takes account of all available information, and I can confirm that the Trust is currently consulting relevant members of staff to prepare a reply. I assure you that this reply will be forwarded to you at the earliest opportunity.

Today 26/10/09 We have still not had a reply from Dr. Throssell that he assured me we would have!

THE 3 QUESTIONS POSED ON MY BEHALF BY UKIP BUT WHICH REMAIN UNANSWERED:
1) Why wasn't my father given his prescribed 'top up', prn prescribed morphine on the afternoon of the 10th January, 2006 and again around 8.00 a.m. on the 11th January, 2006?
(In the 'Sunday Telegraph' article a hospital spokesman said that 'he had indicated he did not have a pain on the afternoon of the 10th’ his penultimate day, although on the previous day it was written in the medical notes that he was 'too ill and unable to have a discussion with staff'. On the 10th at 11.30a.m. the Palliative Care Consultant wrote ‘Tom probably unaware of his surroundings’. Also in the Medical Notes it says on the 9th and 11th that he was in pain.)

2) Why wasn't the syringe driver increased on the morning of the 10th to take into account the 6 extra top ups that he had needed during the previous 16 hours (as is standard palliative care practice) and after after the syringe driver (drip) had
been in situ for 32 hours holding just 10mgs. Morphine?
For the whole length of time that the syringe driver was in situ (55 hours) it was NEVER increased, (as IS standard palliative care practice). It was however doubled on the morning of the 11th after my call to the family G.P and he died 1 hour later. (Doubling of Morphine levels is NOT standard palliative care practice).

3) Why was my father resuscitated on the 7th January 2006 when in the Medical Notes it said DNR (Do Not Resuscitate)?
He was resuscitated to then be allowed to suffer ending up twisting and writhing on his ‘death bed’ until death arrived on the morning of the 11th January 2006.’’

The opening of the debate into Palliative and End of Life Care by eminent Professors and Doctors after the publication of the Patients Association Report.
The Cancer Tsar now having taken up the matter of Care of the Dying by saying training will be Mandatory for staff working on End of Life Care wards

All I would like are some decent, honest answers to some decent, honest questions.
