Thomas Milner's Palliative Care Experience '06!
30/8/10 - Sir Andrew Cash OBE sends us a letter 30/7/10 to summarise our meeting with him 21/6/10. Sir Andrew Cash apologies for 1.Leaving a dying man (40 hours from death) sat in a wheelchair on the MacMillan Palliative Care Ward, Sheffield for 4 hours whilst administrative forms were filled in. 2. Leaving off a convene and pads during the last 2 days of patients life whilst on the MacMillan Palliative Care Ward Sheffield, although the patient was bedridden and haemorrhaging, which meant he was left lying in urine and blood. 3. Not having redressed a large sacral bed sore during the last 2 days of a patients life whilst on the MacMillan Palliative Care Ward Sheffield.
Mind you we must question the NMC - Nursing and Midwifery Council, who in their letter to us 7/7/10 quoted 'staff acted appropriately and within their professional and ethical guidelines'! See our email to the NMC - Nursing and Midwifery Council asking them to explain what is going on at 'NMC - Help or Collusion' under Case Points to the left.
16/8/10 NMC - Nursing and Midwifery Council re your Letter 7/7/10 are you sure that Sir Andrew Cash is now saying to you that the alteration in the Controlled Drug Book, that he conceded to at a meeting with us 21/6/10 regarding the supposed 20.20p.m.entry, should read 08.20a.m.? (See Redacted Dangerous Drug Register at 'Our Evidence" under Case Points to the left).
Now come, come YOU know that this just does NOT stack up with all the other documents that we have! Your organisation and Sir Andrew Cash seem to be getting VERY confused. Have you heard of Denis Healey's First Rule of Hole - when you are in one stop digging! 10/8/10 Sir Andrew Cash OBE and Fellow of the NHS / Department of Health National Leadership Council, Thank You for telling us to get lost and that you will not be answering any more of our communications. We know that this is a standard practice or tactic when the NHS is in a 'spot'! (Sir Andrew Cash OBE's final letter to bereaved Family to be on site asap.)
You have lied and deceived not only us but also our Solicitor, our MP David Willetts and our MEP Nigel Farage. You have covered up your failings by using any method available to you. The fact that D Grade, Junior Nurses and Students were left alone to man an 18 bedded Macmillan Palliative Care Ward is the bottom line and these untrained nurses didn't refill the Morphine Syringe Driver of our Family member which caused him much agony and caused us to have to call the GP for help as he lay dying. Nobody knew what they were doing on that ward - a so called MacMillan Palliative Care Ward!
Let's face it the NHS cannot have proper front line staffing levels when they have the army of 'Managers' and 'Administrative Staff' to pay for, not to mention your salary Sir Andrew Cash OBE of £212.000 p.a.
But at least you did admit to the alterations in the Controlled Drug Book (trying to make it look as though morphine had been administered when it hadn't) which is a Criminal Offence and so Sir Andrew Cash OBE the buck stops with you!
31/7/10 40 days (5 weeks & 5 days) Sir Andrew since we met, are you are stonewalling again! Where's the transcript of our meeting and your response that you promised 23/6/10? Is it the fact that you know we taped the meeting and this has prevented you from sending us more of your 'creative writing'? The concessions of poor care made by Sir Andrew Cash OBE, Fellow of the National Leadership Council, can be seen to the left under Case Points 'Poor Practice Cover Up' - 'CE at STH finally admits to Alterations in the Controlled Drug Book', which is a 'Criminal Offence'. Does that make you a Criminal Sir Andrew Cash OBE?
21/7/10 Sir Andrew Cash OBE GUILTY of misleading David Willetts MP by saying the Morphine Syringe Driver was set up nearly 24 hours later than it was to try to insinuate that there wasn't enough time to increase the amount of pain relief. See 'Andrew Cash Questioned' to left under Case Points AND now Sir Andrew Cash OBE GUILTY of misleading Nigel Farage MEP by saying that 'the Trust has complied fully with the 'Access to Health Records 1990" which is a LIE. See 'Andrew Cash Misleads' to the left under Case Points where we point out Chapter 23 of the 'Access to Health Records Act 1990' which states quite clearly that the 'identities of those involved in the care of a patient CANNOT be withheld'. We know what you are hiding Sir Andrew Cash OBE! We await disclosure of the grades of 7 members of STH staff who have made entries into the medical records of Thomas.
13/7/10 It is now 29 days since our meeting Sir Andrew Cash OBE with you and your staff, which finally took place 21/6/10 4 years and 5 months after the 'Incident'. You did say that you would reply in a letter once the shorthand/minutes of the meeting had been written up. As you know we tape recorded this meeting and are aware of the 'serious concessions' made by the STH. Are you 'stonewalling" us again Sir Andrew, which does seem to be an STH procedure?
6/7/10 The STH have written to our MEP, Mr. Nigel Farage 'finally" confirming that some of the entries in the Controlled Drug Book were made by STUDENTS. However this now shows the NMC up for their collusion as they have written to us stating that ALL the named signatories of the Controlled Drug Book were qualified! See our email about how the 'Cover Up" just gets worse, under 'Poor Practice Cover Up" to the left. It all boils down to the fact that Students and D grade (junior) nurses were left alone to man a MacMillan Palliative Care Ward and that there were just poor Managerial practices. The Sheffield Teaching Hospital Trust and all the 'advisory bodies" just have completely ignored Denis Healey's First Rule of Hole - "When you are in one stop digging"! What the STH NHS has done is 'Cover Up" mistakes to keep the 'Fat Cats" in their superflous positions on the Gravy Train!
26/6/10 We are pleased to announce that during a meeting at the Sheffield Teaching Hospital Trust 21/6/10 with Sir Andrew Cash OBE, CE of STH, Mike Richmond, STH Medical Director, Hilary Scholefield, STH Chief Nurse, Richard Parker, STH Deputy Chief Nurse, Mandy Yates, STH Lead Cancer Care Nurse, Dr. Kay Stewart, STH Palliative Care Consultant and Alison Reitz, STH Palliative Care Ward Manager, several serious failings were conceded. 25/6/10 We are pleased to announce that Raley's Solicitors, Barnsley have confirmed that we will not be called to pay an invoice for the sum of £2.307.08 for payment for the Medical Report as compiled by Dr. Karen Simpson, 'Medical Expert" of Leeds.
You see Dr. Simpson's Medical Report dated March 2009, which outlines when morphine was administered to Thomas just does NOT match the list's and documents that were belatedly passed to us in December 2009 by the STH - Sheffield Teaching Hospital Trust! Someone just isn't telling the Truth! Andrew Lansley, Health Secretary or otherwise known as Secretary of State for Health and his Chief of Staff Jenny Parsons are looking into the matter!
11/6/10 Sheffield Teaching Hospital NHS FoundationTrust sees a DOUBLING in complaints!
2005/2006 - 673 were made an 'official" STH NHS Complaint.
2009/2010 - 1340 were made an 'official" STH NHS Complaint. Where are the 'Lessons being Learnt'?
The Patients Association should be very concerned about this rise in NHS Complaints and should be looking into the NHS Complaints Procedure as a whole. We certainaly have found it to be Self Serving and Corrupt!
10/6/10 We have cordially invited the 'Patients Association', the 'Marie Curie Institute' and 'MacMillan Cancer Care', along to the meeting now finally arranged for June, 2010 with Sir Andrew Cash OBE, 4 and 1/2 years after the death of Thomas - who was Case No. 6 in the 'Patients Association Report 2009" and of whom the 'Marie Curie Institute" wrote had a 'bad death" at the MacMillan Palliative Care Unit, Northern General Hospital, Sheffield. Perhaps some 'Lessons will be Learnt'!
We have also asked for the main participants in this matter to attend the meeting, Dr. Kay Stewart, Palliative Care Consultant, Alison Reitz, MacMillan Palliative Care Ward Manager, and Nurse Katy McKinney - STH - Sheffield Teaching Hospital NHS Foundation has NOT responded to our request! 8/6/10 After being assured by the NMC in April, 2010, that we would be given the grades of the nurses who have made entries into the Controlled Drug Register at Sheffield Teaching Hospital Trust, it appears that the NMC is just part of the 'Game" the NHS play for covering up foul play! Under various 'Acts" we have a Statutory Right to know the grades of McKinney, Bennett, Rowlett and others who have made entries into the Controlled Drug Register on the MacMillan Palliative Care Ward, Northern General Hospital, Sheffield. 3/6/10 We are happy to announce that with the 'unintentional" help of Dr Kay Stewart, Palliative Care Consultant, Alison Reitz Palliative Care Ward Manager, Nurse Gainor Roberts, Nurse Katy McKinney and Nurse Amanda Bennett, we have TODAY finally found out why the 'Cover Up" had to be made over the treatment and care of Thomas, who spent his last 40 hours of life on Sheffield Northern General Hospital's, MacMillan Palliative Care Ward, where the NHS staff really couldn't care less!
After our succinte letter 'STH Misuse of Drugs" 9/2/10 written with the help of a group of Doctors and Nurses (this letter can be seen under Case Points to the left 'STH Misuse of Drugs'), the Sheffield Teaching NHS Hospital Foundation Trust sent another whitewash letter that did not answer any of the questions, points and anomalies raised. However Sir Andrew Cash OBE did offer to meet with us 4 years and 3 months after the death of Thomas. We have written to Sir Andrew Cash to ask why he would like to meet with us - reply - STONEWALLED AGAIN. What games the Sheffield Teaching NHS Hospital Foundation Trust plays!
18/5/10 We are pleased to say that since we issued the above statement on site, the Sheffield Teaching Hospital Trust has finally offered us a date to meet. We shall now be meeting with Sir Andrew Cash OBE shortly.
However in the meantime, we have now been passed photographic evidence of a nurse who has signed out drugs from the Controlled Drugs book as having given medication to Thomas 1 hour before he died.
We NEVER saw this nurse on the Palliative Care Ward and she never attended Thomas. Funny though, as well as all the other alterations and anomalies within the Controlled Drug Register, it turns out this nurse is best friends with the Palliative Care Ward Manager's daughter. The Sheffield Teaching Hospital has been up to no good hasn't it?

A month after Thomas died, 16/2/06, the NHS Palliative Care Unit at the Sheffield Northern General Hospital wrote:
'WE WILL BE ABLE TO REFLECT ON YOUR FATHER'S CASE AND HOPEFULLY IMPROVE OUR SERVICE IN THE FUTURE!!! But they failed to answer our concerns about the pain control that was and was NOT administered. We are still waiting!!! 1/5/10
1/5/10 Nurse Bennett is on Maternity leave at the moment but we have requested that she and the 2 'student" nurses be present at any meeting with Sir Andrew Cash OBE for identity purposes. These 2 'student" nurses had stood in the doorway of the MacMillan Palliative Care Ward 8.00a.m. 11/1/06 and said they couldn't give Thomas anything and that we would have to wait for the Dr's to do their rounds. Our GP was called and that summoned Junior Dr. Julia Morgan to the ward who adminstered morphine and Thomas died 1 hour later. (16/12/09 and we belatedly receive the Controlled Drugs and PRN charts which show that McKinney and Bennett have signed out morphine that was supposedly given to Thomas. We were there NO such interventions took place. We wonder what happened to the morphine?)
21/4/10 The NMC, Nursing and Midwifery Council have been asked to disclose the qualified or unqualified status of 2 'nurses" McKinney and Bennett who have made entries into the Controlled Drug Book, after Sheffield Teaching Hospital NHS Foundation Trust refused to tell us. See 'Our Evidence - Redacted Dangerous Drug Register' under Case Points to the left.
The NMC, Nursing and Midwifery Council told us that Sir Andrew Cash OBE has now offered to meet with us, the family. WHY? Would that be to give us the 'Full and Unreserved Apology" that we asked for in our letter to Sir Andrew Cash OBE, See our letter at 'STH Misuse of Drugs' under Case Points to the left. (We have written to Sir Andrew Cash about this proposal but he has failed to reply to us! Stonewalling is a well practised NHS technique.
We have been asking about the grades of McKinney and Bennett for some months as well as who the witness signature belongs to as NONE of these are on the 'Staff Signature List" as being qualified to give medication. We have been told by the NMC, Nursing and Midwifery Council that Mr. Richard Parker, Deputy Chief Nurse of STH, will be sending the information in a couple of weeks. WE NOW AWAIT MORE ALTERED DOCUMENTS!

Thomas's morphine syringe driver was allowed to run dry on the MacMillan Palliative Care Ward, NHS Northern General Hospital, Sheffield. We had to call a G.P. to summon help and 1 hour later Thomas died.
From then on it has been mistruths, cover - up's, misleading statements and lies. No Apology. Mind you it is the UK - full of Governmant Quango's and corruption!
30/3/10 Toms anguish visits Westminster again and meets with Ministers and NHS Doctors! Tomsanguish looks forward to some serious changes in the NHS and also is interested in new developments in regards to NHS Whistleblowers, whose concerns in the decline of NHS standards were ignored. 16/2/10 We are now happy to announce that Sir Andrew Cash has finally replied to our extensive letter, written with the help of a group of Dr's. and Nurses, which explains how the morphine syringe driver was allowed to run dry and how, when Thomas was 'pulling at the bedsheets', we had to call the G.P. to get anybody to help! DISSAPPOINTINGLY Sir Andrew Cash must have his Politicians hat on, as he just DOES NOT answer any direct questions and it has still not been explained to us why the Dangerous Drug Book/Controlled Drug Register has been altered and why correlating Drug charts just DO NOT match!
Sheffield Teaching NHS Hospital Trust seems to be another NHS Trust hiding behind 'Whitewash Letters" which are the norm when serious mistakes have been made! Never mind, Tomsanguish has heard that there is to be a complete rehash of NHS Hospital Trusts - perhaps Sir Andrew Cash will get a big payoff as is the norm within the NHS when they have made an error! 8/3/10 STH refuse to release requested documents under the Freedom of Information Act.There is no reason for this. The Sheffield Teaching Hospital Trust are saying that divulging the grades of 2 nurses would be a 'breach of confidence and would be unlawful" but that does not make sense as they have already divulged the grades of other staff on the MacMillan Palliative Care Ward, Sheffield.
Details of our evidence will be on site to the left under 'Misuse of Drugs" and 'Our Evidence" shortly! Shame on you Sir Andrew Cash OBE and your 'staff" at Sheffield Teaching NHS Hospital Trust.
1/3/10 The Staff Duty Register that we have, which should list all the names of NHS nurses and and medical staff allowed to give morphine and terminal sedation, does not show the names of the 'nurses" that made entries in the Dangerous Drugs Register,11/1/06. Just who were these 'nurses'. Not being listed on the Staff Duty Register would indicate that they were not qualified to give Morphine! We have asked under a FOIA who were these 'nurses', but the Sheffield Teaching Hospital Foundation Trust just won't tell us. But of course we have seen that the Sheffield Teaching Hospital Trust have altered drug charts and documents and have been less than forthright in this matter and so nothing comes as a surprise to us now, Sir Andrew!
9/2/10 We have now sent a succincte letter to Sir Andrew Cash OBE, Sheffield Teaching Hospital Trust, his colleagues and Members of Parliament. This letter outlines the anomalies found within the Drug Charts and explains fully how the morphine supplied to our Husband, Father and Grandfather was more than halved during the dying process. The Hospital are Stonewalling us again!
5/2/10 - We would like to thank Thomas, Peter, Carol, Violet and Maureen for their help over this matter and leading us to the Truth! STH have passed on to us, mistakenly, the full Dangerous Drug Register which shows the medication given to other patients on the MacMillan Palliative Care Ward, Northern General Hospital, Sheffield!
2/2/10 Morphine signed out, morphine administered the calculations just don't match up!< U>
The 'Misuse of Drugs Regulations 2001" has been contravened. Under the 'Misuse of Drugs Act 1971". Just what documents do we have?
1/2/10 2 'Missing Documents" are requested again under the DPA or FOI.
The 'Staff Signature Register" and clarification of the grades and names of nurses that 'gave" morphine injections. It's been like trying to get blood out of a stone. Come on Sir Andrew Cash OBE only 2 to go!
These documents were requested 31/12/09 and we are still waiting! We had been led to believe that we had a complete set of Medical Notes, having requested them 11/1/06. This contravenes the 'Access to Medical Records Act '.
26/1/10 Drug Charts Destroyed, Alterations to Dangerous Drug Register and Unexplainable Anomalies.
We have now seen 'some" of the Original Medical Notes. Drug Charts and Observation Charts are now disclosed 4 years after Tom's death. Some Drug Charts had been destroyed although the Sheffield Teaching Hospital knew the case was being investigated under the NHS Complaints Procedure.
A serious alteration to the Dangerous Drug Register has been discovered. Mismatching morphine amounts could not be explained. Next Step = It's really up to you Sir Andrew Cash OBE
We have sent an email to the Sheffield Teaching Hospital asking them for Candour and this email can be seen under 'Articles" to the right - " NHS, Lies & Cover Ups'. They are not disclosing 2 important documents that have been repeatedly requested.
25/1/10 Just Can't Get a Straight Answer = NHS Guideline
We have emailed Sir Andrew Cash OBE and his many assistant's to ask them to explain the 'Conflicting Statements" as noted below. Result = Silence as usual. This is standard NHS practice!
14/1/10 Conflicting Statements?
Mr. David Willetts M.P. (Conservative) has now passed us a letter sent to him by Sir Andrew Cash OBE, November 2009. In this letter Sir Andrew says "'I can confirm that when the Healthcare Commission highlighted aspects of Mr. Milner's care which could have been improved, I apologised for these lapses and ensured that the recommendations were acted upon'.
This statement comes in stark contrast to statements issued in the Patients Association Report 2009 and in various press articles. Hilary Scholefield, Chief Nurse at the Sheffield NHS Teaching Hospital Trust issued this statement to the Yorkshire Post 26/8/09 "a review had been carried out by the Trust and the Healthcare Commission about the medication given to Mr. Milner and it was found staff acted appropriately and professionally".
N.B. Regarding the medication, the Healthcare Commission found that the amount of morphine (10mgs.) in the syringe driver was 'Low in the Circumstances'! We NEVER have had an apology!
11/1/10 4th Time of asking to View the Original Medical Records!
It is now exactly 4 years today since Tom died. We have written to Sir Andrew Cash OBE for a 4th time to request to view the original Medical Records held at the STH.
We have been advised that as there are anomalies within some recently disclosed NHS documents in regard to other NHS documents that we have, we MUST examine the originals to see if they have been falsified.
Apparently this is not unusual and we are in contact with other members of the public who have already had their loved ones medical records scientifically examined and they have proved that their medical records HAD been doctored!
7/1/10 Request to view Original Medical Notes
It is exactly 4 years ago today that Tom's NHS 'Terminal Care" began at the Northern General Hospital, Sheffield. We have requested to view all the original Medical Notes held at the Sheffield Teaching Hospital Trust after once again writing to Sir Andrew Cash OBE. Our request has NOT been acknowledged. The Medical Notes we have appear to be different from those that the Hospital are referring to in their correspondence 16/12/09!
18/12/09 Medical Record Entry differs from Hospital's Response
On Tom's penultimate day and just 16 hours prior to death, the lights on the 3 bedded ward were VERY bright! A MacMillan Nurse didn't know how to dim the lights. We fathomed it out - a dimmer switch was outside the ward.
The Palliative Care Consultant wrote 16/2/06 that our only complaint on the evening of the 10th January, 2006 was "the brightness of the lights - an issue that was seen to immediately". An entry into the Medical Records states:"I turned downed the light to their satisfaction".
NOW Sir Andrew writes 16/12/09 - 'You have indicated that you adjusted the lights and I accept that'!
16/12/09 Justifying Mistakes?
3 Months after Dr. Mike Richmond, Medical Director of Sheffield Teaching Hospital Trust telephoned us and said that the Hospital would NOT stand by the times of set up and refill of the morphine syringe driver as listed in Sir Andrew Cash's letter to us, when it was said that we would contact the Press - A document justifying Sir Andrew's list turns up!
This document was NOT included in the complete set of Medical Notes in our possession and does NOT match the times of set up and refill of the morphine syringe driver as noted in the 'Medical Expert's Report" nor in a previous NHS letter. Copies of ALL documents will be on site shortly.

Sir Andrew Cash OBE, Chief Executive, Sheffield NHS Hospital Trust (sth), has sent ANOTHER INCORRECT letter 30/9/09 to us this time! He now says that he cannot answer any of our questions as the matter is with the HEALTHCARE OMBUDSMAN! Where is Sir Andrew getting his information? The matter is NOT with the Healthcare Ombudsman, so come on Sir Andrew let's have some answers!
We have sent ANOTHER letter to Sir Andrew Cash OBE which can be seen to the left under CASE POINTS 'Another Letter to Sir Andrew" asking for some simple answers to some simple questions!!!.

1.Sir Andrew Cash OBE, Chief Executive, Sheffield NHS Teaching Hospital Trust (sth), has still NOT apologised for his ERRONEOUS letter - See CASE POINTS 'Letter from Sir Andrew Cash OBE'. This letter was sent to David Willetts M.P. WHAT ARE THEY TRYING TO COVER UP?
2.The office of our UKIP-MEP Nigel Farage has still NOT RECEIVED a reply to their letter - See CASE POINTS 'UKIP-MEP Letter to NHS'. Three simple questions were asked about care and medication!
3.The Leeds based NHS 'Medical Expert" who invoiced us £2165.83 for her ERRONEOUS Medical Report, has NOT contacted us although we have tried to contact her. Our UKIP-MEP says she must sue for payment or concede that her writings were INCORRECT. HER SILENCE SEEMS TO DENOTE THE LATTER.
8/10/09 The Marie Curie Palliative Care Institute Liverpool has kindly contacted us:
'We believe a good death in our society should be the norm not the exception. I realise that your experience is of a bad death and that there are no words or actions that can change your situation, however your views on the LCP and how we can strive to enable care in the last hours and days of life to be of a high standard for all, would be much appreciated'.

17/10/09, Daily Telegraph - Liverpool Care Pathway (LCP) developers at the Marie Curie Centre defend this method for care in the treatment of the dying patient. They said it is not the LCP that is at fault BUT the Medical Staff who need mandatory training! On the same day, the Government Cancer Tsar steps to the fore saying training of medical staff working on NHS Palliative Care and End of Life Care wards should have mandatory training! Was this just to try to make the NHS/Government look as though they are doing something about the Crisis and Chaos in Palliative Care and End of Life Care?
We thing the whole of NHS strategy is in Crisis and Chaos not just Palliative Care and End of Life Care.
The NHS is the 'National Health Secrets" service, being manned by a plethora of managers and administrators who spend NHS time and NHS expenditure dreaming up efficient sounding titles for themselves. These NHS employees waste time and money to come up with 'new NHS iniatives" and think up 'new NHS healthcare strategies and programmes" that justify their newly created jobs!
At the same time they spend NHS time and NHS expenditure discussing how to cut costs in providing drugs or treatment to the ill or dying.
Blacklisted, suspended or damned if you dare raise your head above the parapet.
Corruption, lies and deceit await, if like us, you dare to complain about a service that is being paid for by me, you and the rest of the UK public.
Regarding Tom's Sheffield Palliative Care LESS experience, 4/9/09 Ms. Christine Beasley, the Chief Nursing Officer and Government spokesperson for the Department of Health, wrote to us saying 'I am sorry that your father did not receive an appropriate level of care at Sheffield's Northern General Hospital'.
At the same time Ms. Hilary Scholefield, Chief Nurse at the Sheffield Teaching Hospital Trust was saying to the press regarding the Palliative Care/End of Life strategy afforded to Tom 'care was appropriate'.
They are not singing from the same hymn sheet. They must be suffering from confusion.
Farce! The NHS should carry a Health Warning. It's got a Medical Condition. It's Elderly. It's got a Terminal Illness.
God Help us All and Shame on Them!
